Senior support leader
Home First look after some of the most vulnerable and frail people in our community who have been discharged from hospital, or for those who experience a short period of ill health enabling them to stay at home rather than be in a hospital bed.
Working closely with our NHS colleagues, we are responsive to the needs of our customers by supporting the prevention of hospital admission and facilitating hospital discharge by providing short-term assessment support to people in their own homes and within our integrated Assessment Hub at Perth House.
Working as part of our valued and vibrant care support team, you'll be supervising a small team of our care support worker colleagues, providing short term support to assess, evidence and enable our customers to maximise their independence to remain or return home. You would be a key player in any absence of our manager in being responsible for the general safety of our Assessment Hub buildings and customers during the night-time hours.
You would be supported by our management team and work closely with social care practitioners and health professionals.
A senior support leader explained:
Derby City Council as an organisation supports staff highly, they have a huge range of training in place to support all members of staff with their job role. There is always room for training if anyone is interested, or feels that another training course would be beneficial to our team. We work well together as team. We all individually have good skills and as a team this makes us even stronger and I must say we do high quality work that's recognised.
If you have professional care and supervisory experience and are looking for a new career pathway or further experience, we want to hear from you. You can email your CV to recruitment@derby.gov.uk and we will be in touch.
