Additionally, we create care plans, and manage payments. We update you on useful services and support families with complex needs. For Shared Lives carers, we match them with people they can support.

- Contact details (name, address, phone, email)
- Date of birth and gender
- Health information (where relevant for support)
- Social care assessments, service and support history
- Financial information to manage payments and check financial help eligibility.
How data is used
- Help you live independently and safely.
- Evaluate risks to your wellbeing.
- Arrange urgent support if needed.
- Provide information and connect to support services.
- Check if you qualify for social care and housing help.
- Create and manage support and care plans.
- Set up and handle payments and community alarms.
- Monitor and support families with complex needs.
- Tailor services to your needs and update you.
Sharing of data
We may share data with:
- health care providers.
- internal council teams when necessary
- other authorised partners and statutory bodies.
Your rights
